Angel Wheels Transportation

provides wheelchair accessible non-emergency medical transportation throughout Chicago’s northwestern suburbs.

 

Availability

  • We will transport 7 days a week, 365 days a year including holidays.
  • All trips need to be booked in advance. We will book trips for the same day as our schedule permits. However, we are not an ‘on-demand’ service. We highly suggest booking in advance since our schedule fills up quickly.
  • Regular office hours are Monday – Friday, 8:00 a.m. to 5:00 p.m.

Book a Trip

 

How much advance notice is needed?

We encourage everyone to contact us as soon as a medical appointment is scheduled. We routinely have trips scheduled 2-3 months in advance. If the medical appointment is changed or cancelled, contact us so we can update your booking.

Same day bookings are accepted as our schedule allows. 

Important details

The following appointment details are needed when booking a trip with us.

  • Name of the passenger

  • Passengers date of birth

  • Date of trip

  • Appointment time (if you need to arrive 15 minutes early to complete paperwork, that is the time we need to know)

  • Pick up address (if the home is not wheelchair accessible, we need to understand the accessibility challenges)

  • Destination address (for hospitals, the hospital name, building name and suite number is all we need)

  • Physicians last name and phone number for an office visit

  • Whether the appointment is for a standard office visit, procedure, test, or admission. This helps us anticipate the return trip timing.

  • Pick up residence phone number

 


Payment

 

Payment

  • Payment is requested at the time of service.

  • Visa, Mastercard, Discover, Personal Check and Cash are all accepted.

  • Bank card information is taken when the trip is called in. We do not retain any useable card data on our systems. Following the completion of the trip, the card is charged, and a receipt will be mailed or emailed to you.

  • Sorry, we do not invoice.

 

Insurance

Does insurance cover the cost of the trip?

Everyone asks and unfortunately, with very rare exceptions, it does not.

While the service is often termed, ‘non-emergency medical transportation’, no medical treatment is being administered (like during ambulance transport). Therefore, medical insurance will not cover it.

Cancellations

Things change -we completely understand.

There is no charge for a cancellation with notice.

 

How much advance notice is needed to cancel a trip?

Simply call and give us as much notice as you have. If it happens to be after hours, leave a message and we will acknowledge your call as soon as possible.
